Le bare portate via sui camion o allineate in una chiesa, le informazioni sulle morti nelle case di riposo, l’intervista a un medico del bergamasco che racconta di essersi trovato a decidere se sospendere le cure ai più vecchi senza aver avuto il tempo per una valutazione clinica accurata, la testimonianza di una farmacista della stessa provincia sulla disperazione dei parenti in cerca – inutilmente – di una bombola d’ossigeno per i congiunti moribondi in casa: sono immagini e notizie ormai note a tutti, che hanno messo a nudo il retroscenadi ageismo – ossia la discriminazione di un individuo in base alla sua età – nascosto dalla retorica dell’amore per i vecchi. Tutto ciò, però, non ha mobilitato da subito l’opinione pubblica, come paralizzata da uno stupore collettivo.
Per una parte di anziani, decisioni di tipo medico ed etico sul loro fine vita sono state prese senza coinvolgere in alcun modo i parenti. Chi ha avuto un prevedibile periodo agonico non breve, cioè chi ha lottato a lungo contro la morte prima di spirare, può non aver ricevuto nessun aiuto per alleviare il dolore. Non c’è dubbio che molte delle figure professionali coinvolte in queste situazioni si siano comportate in maniera “eroica”, a prezzo della loro stessa vita, facendo il possibile e l’impossibile per cercare di salvare le vite degli altri e per umanizzare ogni momento del tragitto, spesso lungo e molto doloroso, verso la morte. Il sistema ospedaliero nel suo complesso si è però trovato impreparato al montare dell’epidemia e ha reagito come poteva: una fortezza assediata aumenta le sue difese, alza i ponti levatoi, sta agli ordini dei capitani.
Molti medici di famiglia, lasciati a loro stessi, hanno curato i pazienti rischiando la vita. Alcune case di riposo si sono trovate sguarnite perché sono mancati improvvisamente i parenti, che andavano anche ogni giorno a orari concordati per assistere i loro cari, e perché il personale, prevalentemente femminile, non aveva mezzi per difendersi dal contagio.
All’inizio della cosiddetta Fase 2 questi eventi drammatici esortano a una riflessione, senza la quale si rischiano altre dolorose repliche. Ciò che è accaduto a una parte degli anziani è un aspetto paradigmatico di una mentalità collettiva, pur da distinguere da specifiche responsabilità istituzionali.
Rsa: contesti a alto rischio, ma decessi evitabili
Dall’inizio della pandemia il gruppo di ricerca International Long Term Care Policy Network sta monitorando, considerandone anche i limiti, i dati sui decessi nelle varie tipologie di case di cura. Limitandoci all’Europa – compresa la Gran Bretagna – è emerso a più riprese che le peggiori performance di mortalità in residenze sanitarie per anziani riguardano la Svezia (a Stoccolma si tratta del 45 per cento delle morti da Covid-19), mentre le meno critiche riguardano la Germania.
Nel documento dell’11 maggio sulle risorse messe in campo dai diversi Paesi, il network internazionale esprime al primo punto una valutazione sintetica: “Mentre sia le caratteristiche della popolazione nelle residenze sanitarie sia la difficoltà di mantenere distanze fisiche quando si vive in spazi comuni significano che gli ospiti delle case di cura sono ad alto rischio di morire per Coronavirus, questi decessi non sono inevitabili”. In altre parole, discendono anche da scelte; esplicite o implicite, prodotte da una diversità di fattori che implicano responsabilità oggettive, mancanza di professionalità a vari livelli (sia organizzativi che assistenziali), ma anche negligenze e omertà. Gli stessi congiunti non hanno reagito per tempo – semmai in ordine sparso – come impauriti dalla prospettiva di doversi far carico di parenti non facili da accudire. Le concause che hanno portato in tutt’Europa a una mortalità anche molto superiore a quella di anni precedenti andranno indagate con attenzione. Tuttavia la convergenza dei risultati tra differenti Paesi (i dati definitivi non sono ancora noti) ci obbliga a riflettere su aspetti culturali molto profondi che hanno portato all’emarginazione del valore della vita anziana. In particolare, della vita anziana più isolata e sofferente.
La Germania reagisce, la Svezia stupisce (in negativo)
Dell’Italia lo stesso network sottolinea i deficit del management, la debolezza delle risorse per cure palliative, la frammentarietà degli interventi ex-post. Questi elementi da soli forse non bastano a indicare una mentalità collettiva. Questa emerge con maggiore chiarezza là dove maggiore è l’organizzazione dell’intervento (o del non intervento).
L’approccio svedese colpisce in negativo perché la rete del welfare per gli anziani e le politiche scandinave di sostegno ai più fragili sono sempre state oggetto di ammirazione. Si pensi ai i modelli avanzati di gender regimes – intesi come politiche sociali volte a scoraggiare la disparità di genere, tanto in famiglia quanto sul lavoro – o all’efficace de-familiarizzazione del care system, dove le necessità di cura dei più fragili non vengono affidate necessariamente ai membri del nucleo famigliare. L’efficienza di questi trattamenti antidiscriminatori stride accanto al numero di morti per Covid-19 nelle case di cura.
I dati e le riflessioni del network mostrano che la Germania – al contrario – ha messo in campo, pur nelle differenze tra Lander, un raccordo virtuoso tra sistema ospedaliero, medicina territoriale e attenzione alle difficoltà del personale. Eppure tanta virtù sanitaria coesiste con la classificazione ufficiale degli anziani in home care nella stessa categoria dei carcerati e dei senza tetto. Gli anziani in Germania sono sopravvissuti in gran numero, molto maggiore che altrove, ma sopravvive insieme a loro uno stigma istituzionale.
L’Italia e i suoi cigni neri
Per l’Italia, il dibattito pubblico ci offre qualche spunto in più per riflettere sull’ageismo che ha attraversato l’Europa. Intervenendo sulle differenze di mortalità a marzo 2020 il Corriere del Veneto giudicava la pandemia un “cigno nero” che ci stava assalendo, sommandosi al già eccessivo carico di anziani della nostra piramide demografica: questo “secondo cigno” costituirebbe la debolezza comparativa italiana rispetto alla Cina.
La generazione di italiani che si è guadagnata per la prima volta nella storia una speranza di vita più lunga – quella dei nostri vecchi – continua a venir considerata un peso di cui ora sarebbe naturale alleggerirci? Il Presidente dell’Istat Gian Carlo Blangiardo ad aprile rassicurava alcuni e forse deludeva altri: la mortalità attesa per il prosieguo della pandemia, sebbene maggiore per gli anziani, non sarebbe bastata per cambiare la struttura della piramide dell’età nella Penisola.
Dall’analisi dei quotidiani italiani tra la prima e la seconda settimana di marzo traspare sollievo per la concentrazione della mortalità negli anziani. È comprensibile, perché l’anticipo della morte in generazioni più giovani è molto meno sopportabile, sul piano biologico e su quello sociale. Tuttavia la medicina sociale, già da due secoli, si è sviluppata per contrastare la selezione naturale che, lasciata a sé stessa, privilegia ancora oggi in maniera selettiva e non propriamente “naturale” alcune classi sociali, alcune età (non sempre la più avanzata), l’uno o l’altro genere e le persone ai margini.
Fino a due generazioni fa si davano per scontate la maggiore mortalità infantile e quella materna. L’Italia, alle soglie della pandemia, ha celebrato l’inutile strage della Grande Guerra senza riflettere sulle morti cumulate, per la guerra e per la febbre spagnola, di giovani maschi mandati al fronte dai padri che rappresentavano le culture allora egemoni. In aprile il tono dell’opinione pubblica è cambiato: si celebrano il dolore dei congiunti e la memoria dei vecchi deceduti. Ma occorre arrivare al 24 maggio perché, finalmente, si legga anche – riferendosi alla Spagna – delle sofferenze dovute alle lunghe agonie di chi è morto da solo, talvolta senza nemmeno aver visto un medico.
Guardando al futuro prossimo, le raccomandazioni di etica clinica in condizioni eccezionali della Società italiana di anestesia analgesia rianimazione e terapia intensiva potranno rassicurarci per affrontare, con sicurezza selettiva, qualche momento critico della Fase 2? È una domanda che accompagna una preoccupazione. Con l’approvazione della Regione Veneto un ragionamento bioetico e tecnico è stato tradotto in criterio amministrativo applicabile: “Può rendersi necessario porre un limite di età all'ingresso in terapia intensiva”.
In Italia è mancato un dibattito sull’etica della cura al tempo (pur eccezionale) del Coronavirus. Il rischio è che questo tipo di linee guida possa intendersi come generalizzabile, che diventi legittimo per via amministrativa in ogni emergenza ospedaliera definita straordinaria. Analoghi documenti francesi e statunitensi sembrano più ponderati, pur nella loro diversità di approcci.
Nonni, donne, stranieri: un groviglio di discriminazioni
Gli anziani sono spesso anche nonne e nonni. Nella Fase 2 i nonni, dopo separazioni segregative, tornano “liberi” e unici responsabili dei rischi maggiori a cui sono soggetti. Molti andranno a sostenere l’occupazione dei figli-genitori, anche a causa di misure di sostegno alla conciliazione lavoro-famiglia sempre occasionali e non sistemiche. E questo incrementerà per loro le occasioni di esporsi a possibile contagio.
Se mancano i nonni, poi, una parte delle coppie con figli piccoli, per ritornare al lavoro, deve scegliere a quale dei due percettori di reddito dare la precedenza. Nella maggioranza dei casi si opta per l’uomo, che recepisce in media uno stipendio più sostanzioso rispetto alla controparte femminile. Non a caso sta aumentando la disoccupazione tra le donne: è la possibile restaurazione del regime mediterraneo dove il maschio è il breadwinner, cioè “porta a casa la pagnotta” occupandosi in via prevalente o esclusiva del benessere economico della famiglia. Così le potenziali vittime di discorsi e pratiche discriminatorie, gli anziani, diventano l’ultimo baluardo contro una discriminazione più antica e consolidata.
Le più svantaggiate restano le donne migranti o di origine migrante, occupate (o disoccupate) nel lavoro di cura. L’ansia per la sostituzione ha due volti. Si teme il peso economico dei troppi anziani rispetto ai pochi bambini “nazionali”, e si vuole evitare che troppi giovani migranti e figli di migranti diventino cittadini europei. Queste preoccupazioni si diffondono anche in Francia e nei Paesi nordici più prolifici. In Italia, riflettendo sulla pandemia, si avanzano proposte di politiche per le donne native volte esplicitamente a “curare” la denatalità della civiltà occidentale con evidente funzione anti-migranti. Aspetti di sessismo e di differenziazione tra i “loro” e i “ nostri” bambini (stratificati inoltre dalle dis-opportunità educative), si aggiungono all’ageismo che stiamo scoprendo.
